Berkshire Healthcare NHS Foundation Trust has this week launched a neurodiversity passport system to help patients who are autistic, or have other neurodiverse conditions, to explain their communication and support needs to staff working for the trust, which can be added to their medical records.
In the initial phase, the passport available is intended for use by adults and older adolescents but a version tailored for children and young people is being designed and will be available soon.
Patients can fill in the form themselves, then print it and hand it to staff at an appointment, or ask a clinician can help them complete it.
